- Routine immunizations can be given at any of the Children’s Clinic locations for regular patients of the Children’s Clinic. The immunizations are given during routine wellness exams. Immunizations can also be given to Children’s Clinic patients* on a walk-in basis during the following times:Monday-Friday, 8:30 – 11:00am, and 1:30 – 4:00pm.
- * Babies must be up-to-date with routine check-ups and older children (age 2-19 years) must have had a check up within one year.

Shot Records
You will be given and official LA State Immunization Record at the time your child receives the shots. Please keep this record for future needs.
If another copy of the shot record is needed at a later date, there will be a $5 fee incurred to obtain it. All shot records can also be obtained at the Health Unit, 3236 Kirkman Street in Lake Charles or 201 Edgar St. in Sulphur.
To request a copy of the shot record, either:
1. call our Patient Services office at 337-478-6480 and leave a message– or –
2. email us with your request.
In your message please leave the child’s name, date of birth, contact information, and how you would like to receive the record (fax, mail, or pick up).
